Liability Waiver (Required for ALL Students)

The registering student has my permission to participate in classes and/or lessons conducted by Amore Art LLC held in classrooms of the Palm Beach County School District or the Martin County School District.

• I understand that participation in this program is at my/his/her own risk and the Amore Art will not be held liable for any industry sustained or be responsible for any medical expenses incurred as a result of my/my child’s participation in this class.

Registration Fees & Cancellation Policies (Required for ALL Students)

This program has three separate parts, you can choose to either enroll your child for the whole year to secure your child’s spot or reenroll at every new enrollment period as you choose.

Spots are limited to 20 kids per class. The fee for each new session is $255, or prorated as indicated where there are fewer than 10 classes within the specified session. This will cover all art lessons/supplies along with their very own mixed media sketchbook that will be theirs to take home and share with you at the end of the session period. Bite-sized candy will be available to provide a little encouragement for hard work and good behavior. Your child will leave with one at the end of each class! Please let me know if your child has any allergies.

The first two weeks will be a trial period for your child to decide if this class is right for them! A prorated refund will be issued if by the end of the second class your child decides this is not a good fit for any reason! I truly want happy dedicated artists and I understand children can change their minds. After the two-week period refunds will not be issued. Thank you for understanding.

• I understand and accept the policies for Amore Art LLC as stated herein and on the Amore Art website and accept responsibility for charges and fees incurred.

Media Waiver (Optional)

I understand that Amore Art may use photographs and/or digital videos of my child’s likeness and/or artwork for the website, local publications, advertising or any other related promotional medium. I waive the right to inspect or approve any images prior to publication and any use to which they may be put. Checking the box on the registration form means you consent to this use. Leaving the box unchecked means you do not consent to your child’s image to be used.

• This waiver is optional, must must be indicated on registration.
